Clinical Dilemma: Saving Teeth or Extract and Implant

Modern clinical practice requires dentists to be familiar with new development in materials, instruments and procedures to guide their patients to the right path to enjoy esthetics and function of dentition through their lives.  Two fundamental clinical dilemma facing dentists today is “save the teeth thru endodontics or extract them and replace with implants”.  Correct answer is not easy.  All depends on which angle you are looking at, from implant or endodontics.  Regardless there has to be a common agreement between dentists what is right thing to do.  In this context many dentists are not aware of tremendous advancement in  Endodontics and but Implants past decades.  In this lecture I will try to develop a consensus what to save and what to extract and replace with implants.  It is utmost important for dentists to be educated the pros- and cons- of the both fields and such that they can guide their patients to the right path.  This is ultimate responsibility of being a dentist.

Challenges in the diagnosis and treatment of persistent post-endodontic pain.”

Endodontists are experts in treating acute pain. However, despite seemingly ideal endodontic treatment, sometimes the pretreatment pain can persist, or a new type of  pain can emerge, creating confusion and suffering for patients, and a real dilemma for clinicians. Some insights from investigations into chronic pain after non-oral surgery will be presented, and related to persistent post-endodontic treatment pain.  Results from an ongoing study on post-operative pain after root end surgery will also be discussed.  Some case-based scenarios will be presented.

Endodonzia innovativa utilizzando PIPS / SWEEPS: aggiornamento

La tecnologia PIPS/SWEEPS viene utilizzata per attivare gli irriganti comunemente usati in endodonzia (NaOCl ed EDTA) e non sostituisce alcuna strumentazione standard. L’utilizzo della punta specifica nella cavit‡ d’accesso, permette una efficace irrigazione anche con minima preparazione canalare ISO 20-25/04.
I protocolli PIPS/SWEEPS sono validati da numerose ricerche e dati pubblicati e non. La microscopia elettronica a scansione, la microscopia confocale e la Micro-TAC sono state utilizzate per valutare la detersione dello smear layer e la rimozione di detriti e tessuto pulpare dal sistema endodontico. Studi batteriologici pubblicati hanno validato l’elevato effetto decontaminante di queste tecniche.
La lezione presenter‡ una panoramica di concetti scientifici alla base dell’applicazio-ne clinica e una serie di casi clinici.

The scientific evidence supporting clinical protocols in Endodontology
Clinical techniques are didactically taught in dental schools, with protocols passed from one generation to the next often based on selected materials and techniques that ‘work in practice’. Recently a series of evidence-based position statements have been published to offer formal guidance for clinical procedures in both Endodontology and Traumatology. Although, these guidelines are evidence-based, it is not always obvious what the nature, quality and volume of the primary research that informs the statements.  This session discusses and analyses the scientific evidence on which recently published position statements for the various clinical procedures are based. The scientific evidence behind the clinical procedures lacking official position statements will also be discussed.

DOTTOR OLIVI
Medico Chirurgo, specialista in Odontostomatologia, Giovanni Olivi è Segretario Culturale Nazionale dell’Accademia Italiana di Odontoiatria Microscopica (AIOM), socio attivo della Società Italiana di Endodonzia (SIE), socio della Società Italiana di Odontoiatria Laser (SILO) e della Società Italiana di Odontoiatria Infantile (SIOI). E’ professore a.c. all’Universit‡ Cattolica del Sacro Cuore di Roma per l’insegnamento di “Laser in Odontoiatria” dove È coordinatore scientifico del Corso di Forma-zione  e Master in “Odontoiatria Laser”. Relatore nazionale ed internazionale Ë autore di oltre 70 pubblicazioni scientifiche e numerosi testi dedicati all’utilizzo del laser:
“I Laser in Traumatologia” (Edizioni Martina, 2010);
“Pediatric Laser Dentistry: a user’s guide” (Quintessence Int.,2011);

DOCTRO GIBBS
Jennifer L. Gibbs, M.S, D.D.S., Ph.D. is Director of the Endodontics Division and Program Director of Advanced Graduate Education Program in Endodontics at Harvard University School of Dentistry.
Dr. Gibbs received her dental degree and her Ph.D. in Pharmacology from the University of Texas Health Science Center San Antonio.  She then completed a certificate in Endodontics and an M.A.S. in Clinical Research from the University of California San Francisco, as well as a postdoctoral training period.  She is now a full time faculty at Harvard.

Dr. Gibbs heads a translational research group focused on understanding the neurobiological mechanisms of orofacial pain with a focus on the nociceptors of the dental pulp.  She teaches clinically in the advanced graduate Endodontic Program and lectures to both post-graduate and pre-doctoral students.  Dr. Gibbs is active in the Faculty Practice at Harvard. She is a past Educator Fellow of the American Association of Endodontics and past president of the Pharmacology, Toxicology, and Therapeutics Group of the IADR.

Josette Camilleri BChD, MPhil, PhD, FICD, FADM, FIMMM, FHEA
Josette Camilleri obtained her Bachelor in Dental Surgery and Master of Philosophy in Dental Surgery from the University of Malta. She completed her doctoral degree, supervised by the late Professor Tom Pitt Ford, at Guy’s Hospital, King’s College London. She has worked at the Department of Civil and Structural Engineering, Faculty for the Built Environment, University of Malta and at the Department of Restorative Dentistry, Faculty of Dental Surgery, University of Malta, Malta. She is currently a Reader and Honorary Specialty Dentist at the School of Dentistry, College of Medical and Dental Sciences, Institute of Clinical Sciences, University of Birmingham, United Kingdom and visiting professor at KU Leuven, Leuven Belgium and University of Oslo, Oslo, Norway. Her research interests include endodontic materials such as root-end filling materials and root canal sealers, with particular interest in mineral trioxide aggregate; Portland cement hydration and other cementitious materials used as biomaterials and also in the construction industry.
Josette has published over 130 papers in peer-reviewed international journals and her work is cited over 7500 times. Three of her publications on the chemistry of mineral trioxide aggregate have been placed in the top 50 most cited papers in International Endodontic Journal. In 2018, she has been awarded the Louis Grossman prize by the French Endodontic Society and is the first female recipient of this prestigious award.
She is the Chief Specialty Editor for Dental Materials in Frontiers in Dental Medicine and a senior editorial board member of Scientific Reports (Nature). She is also an associate Editor for Ceramics and a reviewer and a member of the scientific panels of a number of international journals. Josette is also an international lecturer. She serves on the board of the British Society of Oral and Dental Research as a committee member and honorary assistant secretary, a council member of the British Endodontic Society and is also a member at large for the Dental Materials Group of the International Association of Oral and Dental Research. She is the Editor of Mineral trioxide aggregate. From preparation to application published by Springer in 2014. She is a contributing author to the 7th edition of Harty’s Endodontics in Clinical Practice (Editor: BS Chong) and Glass ionomer cements in Dentistry (Editor: SK Sidhu).
